Who is the service for?
People with heart failure in Islington.
What does the service do?
The Heart Failure Team aims to improve the quality of care for patients with chronic heart failure in Islington. The team supports patients to live with their heart failure. The team consists of a highly specialised Heart Failure nursing team whose work is supported by the British Heart Foundation. The service also works in partnership with heart failure services at the Whittington Hospital and UCLH.
Services offered by the team include:

Heart Failure clinics: The heart failure nurse specialists offer a structured programme to heart failure patients at GP practices.

Heart Failure Support Group: The team is working towards establishing the local heart support group, named – ‘Upbeat’.

Community based heart failure rehabilitation: The team is developing a model of a cardiac rehabilitation based in the community for heart failure patients.

Disease Management: The team helps people to maximise their treatment for Heart Failure including reviewing and titrating medication. People can beseen at home if they are unable to attend a clinic appointment.
How to access the service
Patients are referred to the service by their Islington GP.
