Who is the service for?

The programme is open to young women under the age of 20 years, who at the time of referral are less than 28 weeks pregnant and living in Islington.

What does the service do?

Your own Family Nurse will visit you in your home throughout your pregnancy and until your baby is two years old. We will help you have a healthy pregnancy and a healthy child.

What is a Family Nurse?

The Family Nurses in Islington have been specially selected due to their healthcare backgrounds as either Midwives or Health Visitors. Your Family Nurse’s aim is to recognise your strengths and to build on these enabling you to be the best parents you can be. The Family Nurse will work with you to help you achieve your goals and to improve the health and wellbeing of you and your new family.

How can the service benefit you and your child?

You will have regular visits from your Family Nurse who is specially trained to deliver the programme as well as having a background as a healthcare professional. You will still have your Midwife and Doctor and any other professionals who may be involved with you, however your Family Nurse has special material she can share with you.

Your family nurse will also be able to spend more time with you (about 1-1.5 hours per visit) to discuss needs specific to you and to share the materials with you. You will have individualised care and your Family Nurse will be able to signpost you to the relevant professionals who can meet your needs if she can not help you with a specific issue. How to access the service

Young mothers are referred to the service by their GP, midwife, Connexions or any other support service for young people.

Monday - Friday, 9am – 5pm

Out-of-hours:
The Family Nurse Partnership does not provide an out-of-hours service. If you need advice when the service is closed please call NHS Direct on 0845 46 47. If you need urgent medical help when your doctor's is closed please call Harmoni, the out of hours GP provider on 020 7388 5800. In an emergency please dial 999 or attend your nearest A&E department.
