Islington ELiPSe - End of Life Care and Palliative Care Service

What does this service provide for patients and their families?

Our specialist team provides direct clinical care and works with all people needing palliative care and or end of life care at home across Islington. We work along side your regular doctors, nurses and other support services, such as social services or local hospices and hospitals.

Our team has a variety of registered professionals who you can access including specialist doctors, nurses, occupational therapist, physiotherapist, social workers, and is supported by a fully trained assistant practitioner and administrator.

Islington ELiPSe can help in many ways including:

  • Signposting to services with whom we work in partnership
  • Symptom control for treating troublesome symptoms such as pain, sickness, breathlessness, loss of appetite and fatigue.
  • Emotional and psychological support, especially if you and/or a member of your family is distressed in relation to your illness
  • Rehabilitation helping you remain as independent as possible at home, for example by providing equipment and making adaptations within the home
  • Helping maintain your physical strength, and teaching you strategies to conserve your energy and manage your breathlessness
  • Giving social support to address complex needs including housing and benefits advice, support for your family if you are caring for someone vulnerable or a child.
  • Supporting you to plan your future care especially helping you make decisions that are important to share with your family and health and social professionals
  • Supporting you and your family when the end of life is close
  • Supporting to family and friends throughout your illness especially in bereavement

Outside working hours i.e. during weekends/bank holidays at any time, and overnight during the week, you can contact us in the following ways:

  • If you are at home, you should first try to contact your community nurse or GP
  • If further support is required, we can be contacted either for telephone advice or to request an out-of-hours visit on 0845 155 5000.
  • Callers to this number should ask the switchboard to ‘air call’ the palliative Care Team. We will respond to your call by telephone. If a visit is required we will let you know how long it will take us to attend your home.
