NHS Islington community health services have joined with The Whittington Hospital NHS Trust and NHS Haringey community health services to form an exciting new organisation, called Whittington Health.

Who is the service is for?
Older people and young people with physical disabilities who are eligible for 100% NHS funding. The service also assesses those in nursing beds in Islington to ensure they are receiving the appropriate nursing care and that their needs are being met.
What does the service do?
Nurses within the service set up and monitor programmes of care to suit the needs of their individual patients.
How are patients referred to the service?
Patients are referred to the service from Islington’s Complex Needs Panel or through a "fast track assessment" for those eligible for 100% NHS funding. Patients who become residents in Islington care homes are assessed by a nurse for referral to the service.
