What does the service do?
The Islington Community Rehabilitation Team (CRT) is a community based multi-disciplinary team offering a service to adults with significant physical and cognitive disabilities. We work closely with Islington Council's social services and housing departments, health services and voluntary agencies to provide seamless health and social care to our clients.
We aim to work in collaboration with our clients and their relatives and carers to promote health and optimise independence within the areas of leisure, employment, education and parenting.
Who is the service is for?
We provide a service to adults of Islington with a significant disability. That is:
- Adults with an acquired physical and/or cognitive disability that results in a sudden change of lifestyle that will last longer than 3 months. Examples may include chronic neurological conditions such as CVA, Multiple Sclerosis, Parkinson’s Disease or traumatic injuries, including head injuries.
- Adults with significant congenital physical or cognitive disability and who do not meet the criteria of the Islington Learning Disabilities Partnership e.g. Cerebral Palsy.
- Adolescents who are undergoing a transition into adult services and who need rehabilitation or disability management.
- Adults who have a degenerative condition that is causing rapid deterioration. The condition is likely to result in significant physical or cognitive disability (excluding dementia) in less than 6 months, such as Motor Neurone Disease.
- Adults needing mobility assessments for Blue Badges, Taxicards or Freedom Passes.
How are clients referred to the service?
We accept referrals from:
- GPs
- Hospital staff including consultants, specialist nurses
- Social workers
- NHS and Local Authority therapists - district nurses, practice nurses, community matrons and specialist nurses.
- Health care managers
- Housing support officers
- The Younger Person's Eligibility Panel
- Voluntary agencies
- Clients, relatives or carers

How to get there: Closest tube is Archway on the Northern Line. Buses 41 and 210 pass the centre.
Disabled access: Full wheelchair accessibility including toilets. A hearing induction loop is present in reception.
Opening hours: Monday-Friday, 9.00am - 5.30pm.
Out-of-hours: A message can be left on the answerphone and your call will be returned the next day.
