Behaviour Change and Self-Management
NHS Islington Behaviour Change and Self-Management team have been building together resources for patients and their practitioners to maximise opportunities to support and improve health and wellbeing.
Our aim is to reduce health inequalities through developing self management programmes for patients, training for staff and building other health and wellbeing services for people who want to make health behaviour changes.
How do our service users increase their confidence, skills and knowledge?
EPP (Expert Patients Programme)
This is a six-week course that helps people with long-term conditions to manage their health better and feel more in control. For more information contact Claire, EPP Coordinator, on 020 7527 1209 or epp@islingtonpct.nhs.uk.
Co-Creating Health Self Management Programme
This is a self-management programme which is similar to the Expert Patients Programme. It is a 7 week course for people who have been diagnosed with type 2 diabetes for longer than 1 year. More information.
DESMOND (Diabetes Education and Self-Management for Ongoing and Newly Diagnosed)
This programme is for people who have been newly diagnosed with Type 2 Diabetes. With DESMOND people can find out more about what their diagnosis is and what changes it will mean, and it is an opportunity to meet and share experiences with others. Training for our staff
Supporting Lifestyle Behaviour Change
This is a free full-day training designed to equip staff with up-to-date health promotion knowledge and behaviour change skills to help their clients to lead healthier lives. The training covers the key areas for health promotion for Islington which are healthy eating, physical activity, alcohol and smoking.
Trainees will work towards developing the skills that will enhance client -practitioner interactions by building confidence and skills to raise unhealthy lifestyle topics and motivate and facilitate lifestyle behaviour change.

Co-Creating Health Advanced Development Programme
Co-Creating Health is a national initiative, funded by The Health Foundation, to improve patient self-management of long-term conditions. The Whittington Hospital, NHS Islington and NHS Haringey are working together on this programme alongside local GPs and patients. The programme consist of 3 elements:
- Self-management programme, to help patients manage their long term condition (currently focusing on Diabetes); See flyer
- Advanced Development Programme for clinicians; See flyer
- Service Improvement Programme, changing how care is provided to support patient self-management.
The Advanced Development Programme (ADP) will help participants to:
- Improve skills in motivating and supporting self management in patients with long term conditions
- Learn skills to facilitate change in patients’ self-management behaviours;
- Learn skills in setting an agenda with patients for each consultation;
- Learn to set and follow-up on self-management goals with patients; and
- Make working in partnership with patients a reality
As this is a whole systems approach to fundamentally changing the dynamic between health care professionals and patients and between services and patients, participants on the ADP are required to engage in the other elements of the programme.

This resource is for those who work with Islington residents and want to refer or signpost them onto local services for healthy eating, physical activity, alcohol, smoking, mental health, sexual health, cancer screening, and long term conditions.
It lists all the health and wellbeing promotion and highlights:
• the different types of initiatives currently available
• the location of the service and what client groups they serve
• how to refer or signpost to these services
• plus GP, pharmacy & dentist listings

Daily Health Screens
There are 9 Life Channel TV screens in health centres across Islington, and 1 in NHS Islington HQ.
These are screens located in 9 of our NHS Islington Health Centres. They present public health messages that are health promoting or related to health and well being and reducing health inequalities,.
The messages are relevant to the Islington population and linked to local and national campaigns.
